Updating your personal details
Effective from 1st August 2022.
CHANGE OF ADDRESS
To remain registered at Poplar Grove you must move within our catchment area.
You can check if your new address is in our catchment area here.
If you submit a form and your new address falls outside of our contracted boundary, you will be sent a letter asking you to register at the surgery that covers your new address. You should register with your new GP within 28 days as after that period you will be removed from our patient list.
Please note that it is in your best interest to register with your local practice in your catchment area within the 28-day time frame, because this will allow your electronic medical record to follow on to your new surgery. If we must manually deduct your registration with ourselves, your online medical record will not follow you. This means your new practice will have to wait for your paper records from Primary Care Services England, which can take up to 12 weeks or maybe longer.
CHANGE OF NAME
Please complete our ‘Change of Name’ form. This form is also available in reception. We do request proof of change such as marriage certificate/deed poll.
